The structure of care in most long-term care settings is team nursing. The RN is usually the supervisor and a group of LVNs and CNAs work under the RN. The LVNs are focused on medications and/or treatments. The CNAs are typically the ones who feed, bathe, change, reposition, and transfer the patients. The RNs are usually focused on communicating with the family, outside care providers, and doctors. The RN and the LVN are unable to do a comprehensive assessment of all the patients.
